The Munakata yakuza clan was almost wiped out by assassins from their rival Takeshita clan. One member, Takeshi (Shingo Tsurumi), survived the attack and tried to take revenge on the Takeshita clan, but was arrested and sent to jail.
10 years later, Takeshi is released and decides to search for his daughter, Hinata, on Zeni-jima island. He heads to the island along with what’s left of the Munakata clan, including his former boss (Sho Aikawa). The Takeshita assassins that tried to take out the Munakatas are also on their way to the island in order to recover drugs stolen drugs by one of their former members. However, there has been an improper mixing of the drugs, creating a zombie out-break on the island. Now, the two rival clans are brought back together by fate as the island is overrun by zombies.

組長の宗形博也（哀川翔）が敵対する竹下組に襲われたことで始まった抗争は、宗形組の解散で幕を閉じた。足に傷を負った宗形は運送業を営みながら抗争の最中に逮捕された弟分、武史（鶴見辰吾）の娘・日向（山本舞香）の面倒を見ていた。ようやく武史が出所するが、日向は前科者の父親に会いたくないからと家出してしまう。日向が向かったのは家族との思い出の場所、銭荷島だった。宗形や武史、武史の元妻の桜（鈴木砂羽）は彼女を追って銭荷島に行く。銭荷島には組のクスリを持ち出した竹下組組員の吉田あきら（宮川大輔）が逃げ込んでおり、竹下組の者たちもまた島に向かっていた。島では島民たちの間で咳と高熱が出る謎の疫病が流行。組から盗んだクスリを独自に調合して島民たちに売りつける吉田は自分にもそのクスリを打つが、謎の発作によって死んでしまう。しかし吉田はゾンビとなって蘇り、島民たちに襲いかかったため島はパニックに。かくして宗形たちや竹下組、ゾンビの戦いが始まる。
